Not All Pigmentation Should Be Treated with Laser. Here’s What You Need to Know

Updated: May 9

Pigmentation is not just a spot

Most clients come with one request: “Remove it with laser.”

But pigmentation is not just color. It is a reaction of the skin.

If you treat it without understanding the cause, you can either improve it or make it worse.

1. Sun Spots (Solar Lentigo)

Clear, well-defined brown spots, usually from long-term sun exposure.

Laser suitability:Yes! this is the most predictable type for laser treatment.

What works best:

  • targeted laser treatment

  • precise wavelengths such as 532 nm or Ruby (694 nm)

Alternative / support:

  • daily SPF 50

  • home care with tyrosinase inhibitors (vitamin C, azelaic acid)

  • gentle exfoliation (AHA)

2. Melasma (Hormonal Pigmentation)

Diffuse, often symmetrical pigmentation on cheeks, forehead or upper lip.

Laser suitability: NEVER. In many cases, aggressive laser can worsen melasma.

Why: Melanocytes are unstable and react to heat and inflammation.

What works better first:

  • skin stabilization

  • anti-inflammatory approach

  • barrier repair

Alternative / treatment plan:

  • medical-grade home care (tranexamic acid, azelaic acid, niacinamide)

  • strict sun protection

  • controlled, low-energy laser only if indicated

  • combination protocols, not single aggressive session

3. Post-Inflammatory Hyperpigmentation (PIH)

Appears after acne, irritation, or procedures.

Laser suitability: Possible, but only when skin is calm.

Risk:Too much energy = new inflammation = new pigmentation.

What works best:

  • controlled, low-energy laser

  • correct timing between sessions

Alternative / support:

  • calming skincare (niacinamide, panthenol)

  • barrier repair

  • gradual brightening agents

4. Freckles

Genetic pigmentation that becomes more visible with sun.

Laser suitability: Yes, but results are temporary.

What to expect: Pigment can return with UV exposure.

Alternative / maintenance:

  • strict SPF routine

  • antioxidant skincare

  • periodic maintenance treatments

When NOT to do laser

Laser should be postponed if:

  • skin is tanned

  • there is active acne or irritation

  • pigmentation is unstable

  • skin barrier is compromised

In these conditions, laser can trigger rebound pigmentation.
